Documents Required for Proposal

  • Completed and signed/dated Camp Operators Application
  • Photos or Diagram/Plot Plan of the Camp
  • Completed ACORD application(s)
  • Copy of Operations Guidelines and Safety Manuals
  • Copy of facilities' employment application and Camp Registration & Consent Forms for the Campers
  • Currently valued insurance company loss runs for the current policy period plus 4 prior years

  • These coverages are underwritten by Companies rated A+ (Superior) or better by A.M. Best
